Member Checklist

To complete your application submission, please ensure you have submitted copies of the
following documents:
☐ Birth Certificate for each child needing care.
☐ Documents of Legal custody, if member is legal guardian of child needing care.
☐ Most recent proof wages for the past month for all members of the household.
☐ Official proof of all types of income (SSI, Child Support) for all members of the
household.
☐ Most recent IRS 1040 for all members of the household.
Please Note:
The above list includes the basic documentation needed to complete an application. Depending
on individual circumstances additional documentation may be required.
