Attached are two 10-day letters from the staff of the NCHS Questionnaire Design Research Laboratory (QDRL) (Generic clearance OMB 0920-0222, exp. 06/30/2015). Due to severe time constraints, we have submitted them together as one GenIC.
The first letter describes plans to conduct research to evaluate four sets of health related questions: 1) child disability for use in surveys for the Washington Group on Disability Statistics (WG) with the participation of UNICEF; 2) death rate/life expectancy for the Federal Statistical System (FSS) Public Opinion Survey; 3) health insurance for use in the American Community Survey; and 4) second hand smoke for the National Health and Nutrition Examination Survey (NHANES).
The second letter describes seeks approval to continue to conduct research to evaluate the Blood Donor History Questionnaire for the Office of the Assistant Secretary for Health (OASH) in three additional geographic locations (Austin, Texas; Salt Lake City, UT; Mobile AL/Biloxi, MS).
The total number of burden hours for the two projects is 125 hours with 25 participants at 1 hour each (Four sets of Health Related Questions) and 100 participants at 1 hour each (Blood Donor History Questionnaire Evaluation Study).
