Dear Dr. Schwab:
Attached are two 10-day letters from the staff of the NCHS Questionnaire Design Research Laboratory (QDRL) (Generic clearance OMB 0920-0222, exp. 06/30/2015). Due to severe time constraints, we have submitted them together as one GenIC.
The first letter describes plans to conduct research to evaluate the Uniform Donor History Questionnaire for CDC’s Office of Blood, Organ, and other Tissue Safety (BOOTS).
The second letter seeks approval to continue to conduct research to evaluate the Violence Against Children Survey (VACS) in Malawi in partnership with the Malawi Center for Social Research, UNICEF Malawi, and CDC’s Center for Global Health. The VACS Malawi cognitive test is modeled after the VACS Philippines cognitive test explained to you in a letter dated
July 9, 2012 and approved by OMB on July 22, 2012.
The total number of burden hours for the two projects is 75 hours with 45 participants at 1 hour each (Uniform Donor History Questionnaire) and 20 participants at 1 1/2 hour each (VACS Malawi).
