Dear Resident:

We are writing to ask for your help with an important study being conducted by the Centers for Disease Control and Prevention (CDC). We're gathering information from adults about health. Your phone number was chosen randomly from phone numbers across the U.S. Westat, a health research company, will call you and identify themselves as calling for the CDC. If the call comes at a busy time, please give the interviewer a better time to call you back.

The interview takes about 15-25 minutes and can be completed when Westat calls or at a later time. You can call, toll-free, XXX-XXX-XXX if you have any questions or if you want to schedule a convenient time to complete the interview.

The CDC will use the data collected by this study to develop more effective public health programs.

Your participation is completely voluntary, and you may choose not to answer any question or to end the interview at any time. If you do not wish to participate, there is no penalty or loss of benefit to which you may otherwise be entitled. If you do participate, your answers to survey questions will be treated in a secure manner. No person or household will be identified in any reports from the study.
