I am writing to ask for your help in learning about problems vulnerable Medicare beneficiaries may face
in accessing home health care and the costs associated with treating these beneficiaries. Your
participation is very important in representing the full range of home health agencies working with these
populations and gaining a complete picture of access to home care services for Medicare fee-for-service
patients. The Centers for Medicare & Medicaid Services (CMS) has contracted with L&M Policy
Research, and their partner, Social & Scientific Systems, to gather this information to be used to inform
CMS, Congress, and other policymakers as they consider changes to payments for home health services.
You can help by completing the enclosed questionnaire and returning it to Social & Scientific Systems
(SSS) in the stamped return envelope provided. The questionnaire should take no longer than 15 minutes
to complete, and your answers will be kept confidential. We are interested in hearing from the
administrator of your home health agency or from agency staff in a better position to provide the most
reliable answers, so please feel free to pass the questionnaire along to the appropriate person. If you have
any questions, please call 1-800-XXX-XXXX and ask for the Study Manager, Jane Doe. The staff at SSS
will do everything they can to make it easy for you to participate in this effort.
