Dear Mr./Ms. [Last Name],

I am writing to ask for your help with an important study being conducted for the Centers for Medicare &
Medicaid Services (CMS). The purpose is to learn about any home health service access issues vulnerable
Medicare beneficiaries may have and the costs associated with treating these beneficiaries.
In the next few days you will receive a request to participate in this project by answering questions about
your experiences in providing home health services to Medicare beneficiaries. CMS has contracted with
L&M Policy Research, and their partner, Social & Scientific Systems, to gather this information to be
used to inform CMS, Congress, and other policymakers as they consider changes to payments for home
health services.
Your participation is very important in representing the full range of home health agencies. The
questionnaire should take no longer than 15 minutes to complete. The staff at Social & Scientific Systems
will do everything they can to make it easy for you to participate in this effort.
We thank you in advance for your help in this important study and appreciate your valuable time in
providing input on how vulnerable Medicare beneficiaries can best access the home health services they
need.
