Dear Dr. [Last Name],
I am writing to ask for your help in understanding access issues vulnerable Medicare beneficiaries may
face when referred to home health care. As a physician who treats and refers many of the most vulnerable
beneficiaries to home health care, you are in a unique position to assist us in putting together a completed
picture of these access issues.
The Centers for Medicare & Medicaid Services (CMS) has contracted with L&M Policy Research, and
their partner, Social & Scientific Systems, to gather this information to be used to inform CMS, Congress,
and other policymakers as they consider changes to payments for home health services.
You can help by completing the enclosed questionnaire and returning it to Social & Scientific Systems
(SSS) in the stamped return envelope provided. The questionnaire should take no longer than 15 minutes
to complete. Filling out this survey is voluntary. Your answers to these questions will be kept private and
will not affect your status as a provider. Results of this survey will be reported only in the aggregate; your
organization will not be identified in any way. If you have any questions, please call 1-800-XXX-XXXX
and ask for the Study Manager, Jane Doe. The staff at SSS will do everything they can to make it easy
for you to participate.
In recognition of your effort, we have included a $50 check as an honorarium. We thank you in advance
for your help in this important study and appreciate your valuable time in providing input on problems
Medicare beneficiaries may face when trying to access the home health services they need.
