		The purpose of this survey is to obtain information on the turkey industry. Your answers to the questions below will be used to determine the number of turkeys raised. Under Title 7 of the U.S. Code and CIPSEA (Public Law 107-347), facts about your operation are kept confidential and used only for statistical purposes. Response is voluntary. Please return your completed questionnaire in the postage paid envelope provided or by fax to the number above.
