Title of Clearance: Noninsured Crop Disaster Assistance Program (NAP).

Agency Form Number affected by Change Worksheet: CCC-576, Notice of Loss and Application for Payment Noninsured Crop Disaster Assistance Program.

Other Changes: The form is being changed to streamline a web-based notice of loss process so that producers have a common set of entries for all disaster events regardless of the program using the data. The entries have been reorganized to clarify which are completed when claiming intended, but prevented planting or disaster affected planted acreage. 

The web-based notice of loss process will read data from the FSA-578 so that the producer can identify the prevented or affected acreage.  The acreage identified as prevented or affected, and the COC approved acreage, will be retained in the Notice of Loss system for use by any of FSA’s programs.  

There are no changes to the burden hours in this request. Below shows the details of changes in the CCC-576.

Removed producer phone number and e-mail address.
