Directions: The following questions ask about foods and nutrition, the 5th and 6th Grade Garden-Based Nutrition Curriculum and your experience. There is no right or wrong answer, so please be honest.


  • Were you the primary person responsible for working with the students on the nutrition assignments? If not, who was?




  • Please describe any other food service staff members who participated in the nutrition activities and what their role was.
