Recently a survey titled 2012-2013 Survey of Health Care Providers” seeking information on your attitudes and practices related to contraception provision was mailed to you. You were selected randomly from a list of practicing physicians in the United States.


If you have already completed and returned the survey, please accept our sincere thanks. If not, please consider doing so today. We are very grateful for your help. Results will be used to develop educational materials and tools for providers related to family planning service provision, and to help plan for the implementation of forthcoming national guidance on the provision of quality family planning services.


If you did not receive a survey, or if it was misplaced, please contact Lauren Zapata (770-488-6358) or Marion Carter (770-488-6388), or send an email to: [insert email address], and another survey will be mailed to you.


If you do not provide family planning services to at least 2 women of reproductive age per week, please let us know so we can remove you from our list. As a token of our appreciation, participating physicians will receive a package of provider tools related to family planning service provision at the end of data collection.
