Please mail or fax by the 15th of the month Fax number: 1-775-972-6002			As a service to agriculture and the general public, the Department of Agriculture collects and distributes information on prices being received by farmers for various farm products on or about the 15th of each month. Response to this survey is voluntary and not required by law. However, cooperation is very important in order to estimate accurately the prices farmers are receiving for commodities they sell. The information in your report will be combined with other reporters in preparing estimates. Individual reports are kept confidential. Please note the instructions and mail your report promptly in the enclosed postage paid envelope. Thank you for your cooperation.

INSTRUCTIONS

1. Please report for month when payment is received (both quantity of hay and total dollars paid), not when contract is written or delivery is made, unless delivery date and date of sale coincide.

2. Include sales to any party, except other Nevada dealers.

3. Include sales to any size or type of bale.

4. Report price received at farm gate, excluding freight charges.
