ADDENDUM #2 TO APPLICATION FORM
VCCI REGISTRATION FORM FOR ACCREDITED LABORATORIES
INSTRUCTIONS: Please provide the following information for VCCI registration and submit the
completed form to NVLAP. An example of how to complete the necessary table for your VCCI
registration is provided on page 3.
Also, please use this form in conjunction with NVLAP document LABORATORY SCOPE
EXPANSION REQUEST if seeking a scope expansion request for VCCI Normative
Annex 1: Technical Requirements of Regulations.
NVLAP will then provide VCCI with your laboratory’s facility information necessary to complete
your VCCI-registration. In lieu of the MRA signed in February 2007 between the United States
and Japan on mutual acceptance of Electromagnetic Compatibility and Telecommunication
testing results provided by accredited testing laboratories, NVLAP entered into an MOU with
VCCI signed on December 13, 2006. The MOU states that NVLAP agrees to provide VCCI with
the following information:
“NVLAP shall provide VCCI with the following information for each Accredited Laboratory in an
appropriate way. . . . They are, the accreditation number, name of the laboratory, name of the
measuring site of the laboratory, type of measuring facilities(*) and scope of the accreditation.
Also NVLAP shall promptly provide VCCI with pertinent information in an appropriate way when
there have been changes in the substance of the accreditation such as reduction of the scope,
nullification, temporary suspension and relinquishment of accreditation.
* Note: Types of measuring facilities include facilities to measure radiated EMI, main ports
conducted EMI and communication ports conducted EMI.”
