CDC Plans to collection information from awardees funder under the community transformation grant (CTG) program. Awardees will report progress and activity information to CDC unsing an electronic Management information system(MIS). Reports are due twice per year but updates can be enetered at any time. OMB approval is requested for three years.

No changes to the content of the information collection are proposed at this time. There are no changes to the estimated burden per response; however, there will be an increase in the overall burden due to the addition of the new CTG awardees.
