CDC Water and Health Survey – Last Chance to Participate!

Over the past few weeks, we have sent you a series of mailings about an important study on water use and health. The study will provide us with information on how people use their tap water and help us keep it safe and healthy. Hearing from everyone we contact assures us that we understand each and every person’s unique experiences.

You can also fill out one of the paper surveys we sent and return it in the postage paid envelope we included with the survey.
Please know that, although we hope you decide to participate, your response to this study is completely voluntary. If you are not a customer of [water utility XX], were not at home during the time period we ask about in the survey, or do not wish to participate for other reasons, please let us know by returning the blank survey with a note indicating so. If you prefer, you can call us at 1-404-639-1700 or send an e-mail to [email protected] to let us know that you do not want to participate (please be sure to include the code number from the first page of your survey). Letting us know that you do not plan to participate would be very helpful.
