MAPD: pg. 31; PDP: pg. 31	MA-PD: 3.1.1; PDP: 3.1.1	CMS requires that applicant attest that it doesn't have any covered persons that were part of a CMS contract after June 6th, 2011 that non-renewed or terminated.	While we have access to organizations we have non-renewed/terminated, we don't have that same information for other plans. We request that CMS provide a list of contracts that fit the description of these conditions.
