Your help is needed in providing prices received for hay for the previous full month and the current partial month. Please fill out the items which apply to you and return it in the enclosed self-addressed, stamped envelope. Under Title 7 of the U.S. Code and CIPSEA (Public Law 107-347), facts about your operation are kept confidential and used only for statistical purposes in combination with similar reports from other producers. Response to this survey is voluntary. However, your help with this information is greatly appreciated.

According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a valid OMB control number. The valid OMB number is 0535-0003. The time required to complete this information collection is estimated to average 10 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.

INSTRUCTIONS
1. Please report for month when payment is received (both quantity of hay and total dollars paid), not when contract is written or delivery is made, unless delivery date and date of sale coincide. 2. Include sales to any party. 3. Include sales of any size or type of bale. 4. Report price received at farm gate, excluding freight charges.
