Pre-test Telephone Interview Script

Good Morning/Afternoon! This is ______________________ (name) from the Division of HIV/AIDS Prevention at the Centers for Disease Control and Prevention (CDC). I’m calling regarding our appointment to pre-test for the biomedical needs assessment survey we discussed earlier. Thank you for agreeing to participate in this pre-test. Your comments will help us make this a better needs assessment so we can provide better trainings and tools to CBOs.

While we’re on the phone together, you’re going to take the survey via the Survey Monkey link (Att 9a Assessment Survey).

As you go through the survey, please let us know if you have any questions about any of the items on the survey. Please let us know if the questions or answers do not seem clear. Also, please let us know if there are any questions you feel that we should have asked.
