FRA is updating the PDF fillable format version of Form FRA F 6180.49A to make this form more user friendly for respondents. Specifically, FRA has added a blank space after number 7 (Safety Appliance) of *15 Item codes in case an additional item code (number 8) is needed to capture the correct data for item number 15 of the form. Also, FRA has modified item number 22 to include both dates and text so respondents can fully complete this field. In the previous version of the PDF fillable form, respondents could only put in the test date and not the place because this data element did not allow for text. NOTE: The form itself has not changed, and the time to complete the form has also not changed.
