Electronic Questionnaires for Investigations Processing (e-QIP)
April 2006 Standard Forms Revision

Instructions for Reviewing Screen Samples
Contained within this directory are PDF representations of the e-QIP implementations of
Standard Forms 86, 85, 85P, and 85PS. This includes the printed representation of the eQIP screens used by the applicant for login, selection of an investigation request, form
editing, request certification, and request release. Also included are examples of the
Review Copy, Archival Copy, and Signature Forms artifacts generated by the system.
To review these artifacts in the natural order, first start with Screen Set 1, which covers
steps leading up to the point where the applicant begins editing the form. Next, select
Screen Set 2 for the desired form to review. Screen Set 3 includes screens that the
applicant encounters after he or she finishes the form editing process, and continues
through the certification and release process.
The Review Copy document represents the web page that e-QIP generates when the
applicant clicks the “Display” link. This document is available to the applicant at any
time during form editing, up until the point when he or she certifies the investigation
request. After certification, the system generates the Archival Copy and Signature Forms
documents.
Included in the Web Pages directory are the web page representations of all screens
included in the Screen Set documents. View these in Microsoft Internet Explorer or
Mozilla Firefox for best results. The directory named “static” is used for supporting the
web pages. It contains no content for review.
Note: Viewing certain screens using Internet Explorer may cause a "restricted file from
showing active content" message to appear in the web browser window. This is normal
and may be ignored.
