Honesty Pledge

HONESTY PLEDGE

Honesty Pledge (immediately after consenting) [text to go on study website]

During the QuitTXT study we will be asking you questions about your smoking and your interest in quitting. It is very important that your answers are truthful and honest, especially about how we can contact you. True, honest answers are the only way we can receive accurate information to help improve this program so it can help others.

Here’s why being honest is important:

  • Accurate responses to survey questions will allow us to see whether the program actually helps people in their attempt to quit smoking.

  • Providing honest information about yourself (such as your age, income, education) allows us to know what types of people benefit most from this program and whether it need to be changed.

  • Accurate contact information will allow us to send you study information, including surveys and your thank-you gifts in a timely manner.

Please take a moment to read and agree to provide honest answers.

Please contact Linda Squiers at [project phone number] if you have any questions.

I pledge on my honor that the answers submitted for this survey are true and honest to the best of my knowledge and ability. I pledge that I have truthfully represented my name and age. I pledge that I will answer all future surveys for this study truthfully and honestly.”
