MAC Satisfaction Indicator (MSI) Participant Information Registration Form
CMS understands the concerns that providers have with the services of their MAC. For this reason, we will administer a provider satisfaction survey each year to give providers an opportunity to give their feedback. However, the comment received Eddy Roumen from Advocat- Diversicare for FR Collection Document identifier CMS-10457 (OCN 0938-New) is a provider complaint about a MAC. It is not a comment about MSI Participant Information Registration form. Therefore, it is irrelevant to this new collection. As stated in the supporting statement, this form is not a survey. The purpose of the form is to collect accurate provider contact information for administering the survey. CMS is not required to make any changes to this collection based on the comment received.
