Revision to the Collection Instrument:

We are adding Forms HA-85, Request to Withdraw a Hearing Request, and HA-86, Request to Withdraw an Appeals Council Request to give the claimants an easier way to submit a written record of these requests. Currently, claimants and representatives have no standardized form with which to make a withdrawal request. Adoption of these new forms will help ensure such requests conform to regulatory and policy requirements.
