The Centers for Disease Control and Prevention (CDC) and their nonprofit contractor, RTI International, are conducting an important study about local preventive health activities. We need your help to learn about the health of adults and children in your community.

Participation involves:

• a couple of minutes to tell us a little about your household and to see if your household is eligible to participate further;

• a telephone interview with a selected adult and then a home visit to collect physical measurements;

• an interview about a selected child and physical measurements of that child, if your household includes at least one child between the ages of 3 and 17;

• a gift of $60 for full adult participation and $10 for child participation.
