Circumstances of Change Request for OMB 0920-0009


This is a change request for #0920-0009, which received a three-year extension through 04/30/2016 for the collection of case reports for 16 different diseases from 57 reporting jurisdictions (50 states, 2 cities, and 5 territorial health departments) using case report forms.


Each year, the Council of State and Territorial Epidemiologists (CSTE) establishes the public health surveillance priorities for the nation and policies which are voted on by the Chief Epidemiologist in each U.S. State and Territory. CSTE approved re-instatement of leptospirosis on the Nationally Notifiable Conditions (NNC) list, effective January 2013, as the incidence appeared to be increasing, new risk groups had been identified, there was potential for outbreaks to go unrecognized, and technology was no longer a limiting factor. Leptospirosis is considered a re-emerging disease across species – both human and animal – and is one of the most widespread zoonotic diseases in the world; it was a notifiable condition in the US until 1994.


In response to the reinstatement of leptospirosis as a NNC, the Bacterial Special Pathogens Branch has developed a case report form to collect disease-specific information from state health departments. At the national level, the notification of leptospirosis cases to CDC will be used to define and describe the burden of reported leptospirosis infections in the U.S., information which is critical for measuring disease trends, assessing the effectiveness of prevention and control measures, identifying populations or geographic areas at high risk, developing public health policies, formulating prevention strategies, and allocating resources.


The addition of leptospirosis case report collection to package 0920-009 will allow for data collection to occur during 2013, as requested by CSTE. CDC is working to streamline efforts and reduce burden by combining all notifiable diseases into one OMB package, 0920-0728, National Notifiable Diseases Surveillance System (NNDSS). This is expected to happen in 2013-2014; at that time, leptospirosis will be rolled into 0920-0728 along with most of the diseases in the 0920-0009 package.



Burden


The annualized burden hours and cost to reporting jurisdictions to submit this data to CDC will not change significantly from the original estimates in the “Estimates of Annualized Burden Hours and Costs” section in A.12 of the supporting documentation for OMB #0920-0009. The change to the annualized burden hours and cost is minimal because the reporting jurisdictions are currently collecting leptospirosis case data for internal state purposes. Therefore, the effort to submit case report information to CDC does require a minimal cost in hours; however, the estimated burden of disease is low and the amount of information requested is limited.
