Form Changes Justification

Justification for Non-Material Change for 1505-0247

This request captures several minor changes to the originally cleared form for this information collection. There is no change to the information being collected or the burden imposed on the public. The changes to the form are as follows:

Part I, question #1 was moved to the front of the survey next to the SBLF# blocks.

Part I, questions #2 and #3 were combined, edited slightly and moved to Part IV, question #19.

Part II, question #4 was edited slightly and moved to Part I, question #2.
