Instructions to FCC Form 498
Purpose of this Submission: This submission is being made for proposed non-substantive revisions to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to revise FCC Form 498 and its accompanying instructions.
Summary of Proposed Revisions to FCC Form 498 and the Form 498 Instructions
General Information – FCC Registration Number or CORES ID: The Form 498 was revised at page 1 by replacing “FRN Number” with “FCC Registration Number (CORES ID)” to clarify what information is required. The same change was also made in the Form 498 Instructions at page 5.
Rural Health Care Program – Offset Indicator: In the Healthcare Connect Fund Order (27 FCC Rcd 16678), the Commission changed the offset procedure from mandatory to voluntary for telecommunications companies participating in the Rural Health Care Program. The Form 498 was revised at page 8 (Line 122) to separate the Rural Health Care Program offset indicator from the Schools and Libraries offset indicator. The Form 498 Instructions were revised to remove at page 10 the mandatory offset requirement and to add at page 14 the separate offset indicator for the Rural Health Care Program.
Rural Health Care Program – Certification: In the Healthcare Connect Fund Order, the Commission required that service providers (vendors) participating in the Healthcare Connect Fund must certify that they will provide to health care providers, on a timely basis, all information and documents regarding the supported service(s) that are necessary for the health care provider to submit the required forms or respond to FCC or USAC inquiries. These associated information collections and forms are under OMB Control Number 3060-0804. The Form 498 was revised to include at page 8 (Line 23) a certification applicable to service providers participating in the Healthcare Connect Fund. The Form 498 Instructions were also revised at page 14 to include this certification.
Authorized Contact Signature – Definition of Officer: The Form 498 Instructions were revised at page 18 to include a definition for “officer.” This change was made to clarify who may sign the form.
