Subject Line: Survey About Smoking and Health

You have been selected to participate in a research study about your attitudes and beliefs toward smoking and health because you are a [FILL IF KN PANEL -- Knowledge Networks panel member] [FILL IF OFF-PANEL – potentially eligible study participant]. The study is sponsored by the Centers for Disease Control and Prevention’s Office on Smoking and Health and contains up to two questionnaires. Your participation in the study is voluntary. Each questionnaire will take about 25 minutes to complete. If you choose to participate, please answer the questions honestly and in a private setting. You can earn up to 25,000 bonus points for your participation in this study.

We recently sent a request for you to complete a questionnaire. The information collected will be used to help us learn more about smoking and health. The questionnaire will take you about 25 minutes to complete. Your survey responses will be maintained in a secure manner. Your survey answers will not be shared with anyone outside the research study.
