TRUCKING SURVEY ACKNOWLEDGMENT
This survey is being used to gather opinions and information which will help the Federal Motor Carrier Safety Administration make important policy decisions. Some of the questions are sensitive in nature, and relate to regulations and compliance with them. You may wonder if answering them truthfully will put your job at risk. Be assured, all of your answers will be confidential, and will not be shared with anyone, except when grouped with other drivers. Your identity will never be disclosed to the FMCSA, and your company will never know you participated.
Your participation is voluntary; it is not mandatory.
