The Maternal and Child Health Bureau is conducting a survey to determine how much of your state’s federal fiscal year 2011 federal Title V Maternal and Child Health Formula Block Grant funds were spent reimbursing providers for health services to infants and children, including children with special health care needs, and pregnant women.
Our contractor, RTI International, will contact you via e-mail within the next 2 days to send you the survey. RTI will also hold a 30 minute conference call on <Day, Month, Year> from <Time> to <Time> to answer any questions you may have about completing the survey. The call-in number is <Phone Number>.
