Dear Mr./Mrs. «LastName» <>:
Thank you for your continued participation in the Agricultural Health Study
(AHS). You are an essential member of this study. The involvement of
people, just like you, helps us learn about how agricultural and
environmental factors may affect the health of farmers and their families.
Occasionally, we contact everyone in the AHS to update information
about their health. Enclosed is a brief questionnaire about your health as
well as a contact update form for you to complete. These are separate
documents to help us protect your privacy. If you have any questions or
need assistance completing it please call us at 1-855-443-2692 or see our
survey website, www.aghealthsurvey.org.
Also enclosed is a copy of the most recent AHS Study Update. We hope
you enjoy it and learn something new about the study.
Even if you are retired or no longer farming, your involvement is very
important to us. Your participation in this or any activity of the AHS is
voluntary. You remain in the Agricultural Health Study even if you do not
participate at this time.
Again, we want to thank you for your assistance in making the Agricultural
Health Study an important and successful study in the agricultural
community.
Sincerely,

Jane Hoppin, ScD
