CHIPRA Physician survey reminder letter
MATHEMATICA LETTERHEAD
Dear [FIRST NAME] [LAST NAME]:
Mathematica Policy Research has been trying to contact you about an important survey sponsored by the Agency for Healthcare Research and Quality (AHRQ) and endorsed by the American Academy of Pediatrics (AAP) and the American Academy of Family Physicians (AAFP). We are asking family medicine physicians and pediatricians their opinions about the value of using quality measures and health information technologies to improve the quality of care for children. We expect the results of the survey to be informative and influential in helping policy makers understand how best to collaborate with the pediatric community on issues related to quality measures and quality of care.
Unfortunately, we have not yet received your completed survey. Your responses will play a vital role in the accuracy of the results – by responding, you will help maximize the chance that the results are representative.
Whether or not you or your practice uses quality measures, we need to hear from you. For convenience, we have enclosed another copy of the survey along with a postage-paid return envelope. We will soon be ending data collection – please consider completing the 15 minute survey today. Your answers will be kept confidential to the extent permitted by law and findings will not be reported in any way that identifies you or your practice.
If you have any questions about the survey, or if you prefer to complete the survey by telephone, please contact me at (XXX)XXX-XXXX or [email protected].
Thank you in advance for your participation in this important study.
Sincerely,
