The Agency for Healthcare Research and Quality (AHRQ) is sponsoring a national study to better understand how quality improvement measures are used by practices providing care to children covered by Medicaid or CHIP. In addition, AHRQ would like to understand the kind of quality improvement feedback physicians seek in order to improve the quality of pediatric care.

You have been randomly selected for participation in this important study of primary care physician who provide care for children. Within a couple of weeks, Mathematica Policy Research (our contractor for the survey) will send you the survey in the mail. The survey should take about 15 minutes to complete. It is very important that we hear back from you; your response will allow us to obtain an accurate and unbiased picture of how quality improvement measures are impacting the delivery of pediatric care.

Your answers will be kept confidential to the extent permitted by law and results will be reported in ways that do NOT identify you or your practice.
