Revisions to Form CMS-R-70 Supporting Statement for the Information Collection Requirements in HSQ-110 Acquisition, Protection, and Disclosure of Quality Improvement Organization (QIO) Information and Supporting Regulations in 42 CFR Sections 480.104, 480.105, 480.116, and 480.134

	3	Section B1	Revise as follows:	Replace:  B1 entire last paragraph revised With: B.1 To protect the rights of individuals and providers, QIOs will give a notice to providers and/or individuals, which will contains a description of the type of data QIOs will be collecting, maintaining, and disclosing about them. The QIO will also give the title and address of the QIO employee whom is responsible for maintenance of this data so that the individual or provider will know whom to contact at the QIO. This requirement ensures that the public will be fully informed about the type of information the QIO maintains on them.	Editorial

	6-7	Section 480.104	Revise as follows:	Remove: The only burden to the QIOs is the time to initially develop the form letter. Also delete any additional language pertaining to the QIOs developing an initial form letter. Add: Since the QIOs already have a form letter, there will be no administrative burden on them for that requirement.	The 3 new QIOs already have developed a form letter so it will not pose a time burden on them to develop a new form letter. At this time, there are no new QIOs projected to form.
	8-10	Section 480.116	Revise as follows	Remove: Any language pertaining to the 3 new QIOs needing to develop a new procedure. Remove any dollar amounts and hours pertaining to the QIOs developing new procedures. Add: Since all the QIOs have already developed these procedures, there will be no administrative burden for this section.	The 3 new QIOs already have developed procedures to provide information about QIO information collection and maintenance to patients, practitioners, and providers coming under review so it will not pose a time burden on them to develop a new form letter.
