Dear (Ms./Mr.) Last Name:

We have been trying to reach you for an important study we are conducting on behalf of the U.S. Department of Labor (DOL). You were scientifically selected from among people in your state who recently filed for unemployment insurance (UI) benefits to represent other UI recipients. You cannot be substituted with someone else. We need to interview you even if you did not receive or are no longer receiving benefits. Regardless of whether you are currently working, retired, or unemployed, we would like you to participate in the survey.

Your participation is very important! Participation in the study provides you with a unique opportunity to share information that will help DOL and policymakers assess how well the UI program is serving the nation’s workers and refine it to better meet their needs. You can receive up to $90 for participating in this study. The survey can be completed on the web or on the phone any time that is most convenient for you.

To complete by web

  • Log on to https://www.xxxxxsurvey.xxx and enter the following case-sensitive login information. The website is available 24 hours a day, seven days per week.

Username: [XXXX]

Password: [XXX]

To complete by phone

  • Call Mathematica toll free at 1-800-XXX-XXXX to complete your survey with an interviewer. The interview can be scheduled for any time that is most convenient for you -- 7 days a week during daytime and extended evening hours.

We look forward to speaking with you. Thank you in advance for your participation.

Sincerely,

An Affirmative Action/Equal Opportunity Employer
