REMINDER EMAIL

From: U.S. Department of Labor Study

Subject: U.S. Department of Labor Study about the Experiences of Unemployment Insurance Recipients

Dear First Name:

We recently invited you to take part in a study being sponsored by the U.S. Department of Labor (DOL). The purpose of the study is to learn more about the experiences of people who recently applied for unemployment insurance (UI) benefits. You can participate even if you are currently working, retired, or unemployed. You can receive up to $90 for participating in this study.

An interviewer from Mathematica has tried to call you to complete your interview but has been unable to contact you. You were scientifically selected for this study to represent other UI recipients in your state. You cannot be substituted with someone else, so it is very important that we reach you. Participation in the study provides you with a unique opportunity to share information that will help DOL and policymakers assess how well the UI program is serving the nation’s workers and refine it to better meet their needs.

The interview can be completed on the web or on the phone at any time that is most convenient for you.

To complete by web:

  • Log on to https://www.xxxxxsurvey.xxx and enter the following case-sensitive login information. The website is available 24 hours a day, seven days per week.

Username: [XXXX]

Password: [XXX]

To complete by phone:

  • Call Mathematica toll free at 1-800-XXX-XXXX to complete your survey with an interviewer. The interview can be scheduled for any time that is most convenient for you -- 7 days a week during daytime and extended evening hours.

We look forward to speaking with you. Thank you in advance for your participation.

Sincerely,

Julita Milliner-Waddell
