Asiana Airlines records indicate you were a passenger on flight 214 on July 6, 2013. As
you may know, the National Transportation Safety Board (NTSB) is an independent federal
agency authorized by Congress to investigate and determine the probable cause of transportation
accidents. The NTSB issues reports and makes recommendations to other federal and local
agencies and to the aviation industry to prevent accidents and to prevent injuries caused by such
accidents.
Enclosed is an NTSB passenger questionnaire, which contains information concerning
how you can return the questionnaire to me. If the NTSB has an e-mail address at which we can
send you correspondence, then you may have received, or might receive in the near future, an email message containing the questionnaire. Completion of the questionnaire is voluntary, but we
sincerely hope that you will take the time to complete it. Your completion of this detailed
questionnaire will greatly assistthe NTSB in evaluating the facts, conditions, and circumstances
surrounding the accident.
Additionally, we are interested in obtaining copies of any photographs or video footage
taken by passengers during or after the evacuation. These items often prove invaluable to our
investigations. If you have any photographs or video footage you have not already provided to
the NTSB, or would like to relate your experience via telephone, please contact me directly at
