We are writing to invite you to take part in a study. The study will help researchers learn how to
improve health care for patients like you. This letter describes the study.
Your doctor’s office, [PRACTICE NAME] is taking part in a program called the Multi-payer
Advanced Primary Care Practice (MAPCP) Demonstration. That program was created to help
improve the care that [PRACTICE NAME] and other doctor’s offices give to patients.
The Centers for Medicare and Medicaid Services (CMS) is the federal agency that pays for the
MAPCP Demonstration. CMS wants to learn whether patient care is improved because of the
MAPCP Demonstration. That is why CMS is working with researchers from RTI International,
the Urban Institute, and The Henne Group to conduct small group discussions called “focus
groups.” The focus groups will let us hear from patients like you.
To participant in these focus groups, you must be currently enrolled in Medicaid and at least 18
years old. If you are younger than 18 years old, we would like to invite to a focus group your
parent, guardian or caregiver who helps you with your health decisions or takes you to doctors’
appointments.
If you would like to be in a focus group, please call so we can first ask you some questions.
That focus group will take about two hours of your time. If you are eligible and join the focus
group discussion, we will give you a $50 gift card.
