We are writing to invite you to take part in a study. The study will help researchers learn how to
improve health care for patients like you. This letter describes the study.
Your doctor’s office, [PRACTICE NAME] is taking part in a program called the Multi-payer
Advanced Primary Care Practice (MAPCP) Demonstration. That program was created to help
improve the care that [PRACTICE NAME] and other doctor’s offices give to patients.
The Centers for Medicare and Medicaid Services (CMS) is the federal agency that pays for the
MAPCP Demonstration. CMS wants to learn whether patient care is improved because of the
MAPCP Demonstration. That is why CMS is working with researchers from RTI International,
the Urban Institute, and The Henne Group to conduct small group discussions called “focus
groups.” The focus groups will let us hear from patients like you.
We are sending this invitation to you because you are enrolled in Medicare or both Medicare and
Medicaid and received care from a practice participating in the MAPCP Demonstration. If you
would like to be in a focus group, please call so we can first ask you some questions. That
focus group will take about two hours of your time. If you are eligible and join the focus group
discussion, we will give you a $50 gift card.
If you are unable to come to our focus group, but have a caregiver who helps you with your
health decisions or takes you to doctor appointments, we would like to invite him/her to a focus
group.
