Indicate the quarter this form is being submitted.
Year
Indicate the year this form is being submitted.
New QR or Correct / Update
If you are submitting this QR for the first time, you should select "New QR". If you are updating or
correcting a previously submitted QR, you should select "Correct / Update QR". If you select "Correct/
Update VERR", you must enter the corresponding quarter for the QR that you are updating/correcting and
the reason for the correction / update in the "Notes" field.
Quarter Report Number
Please enter the applicable quarter for the QR. This is only required for six consecutive quarters after the
recall was started.
EPA Voluntary Emission Recall Report (VERR) Number
You must enter the number for the VERR that this quarterly report (QR) corresponds to. This number
should be the number for the original VERR. Accordingly, this number should be exactly four digits and
must contain zeroes for numbers with fewer than four significant digits (e.g., "0007"). If you are unsure
of the original VERR number, you can also use the first four digits of the number for an updated/corrected
VERR with an eight digit number (e.g., "0007-R01 => "0007").
Additional Owner Notification this Quarter?
Indicate if you have performed additional notifications, following the initial notification at the start of the
recall campaign, to owners of the vehicles involved in the recall campaign for this quarter.

Enter the number of vehicles that were inspected and found to be ineligible for repair due to improper
maintenance as outlined by the criteria in the recall campaign.
Number of Vehicles/Engines Unavailable for Inspection/Repair - Due to Exportation
Enter the number of vehicles that were unavailable for inspection or repair due to exportation.
Number of Vehicles/Engines Unavailable for Inspection/Repair - Due to Theft
Enter the number of vehicles that were unavailable for inspection or repair due to theft.
Number of Vehicles/Engines Unavailable for Inspection/Repair - Due to Scrapping
Enter the number of vehicles that were unavailable for inspection or repair due to scrappage.
Number of Vehicles/Engines Unavailable for Inspection/Repair - Due to Other Reasons

Enter the number of vehicles that were unavailable for inspection or repair due to a reason, or reasons,
other than exportation, theft, or scrappage. Also, please specify the reason(s) in the "Notes" column.
Notes
Use the notes column to enter additional comments related to the quarterly report. Examples may
include, but are not limited to reasons why vehicles were unavailable for inspection or repair, reason for
an update/correction of a quarterly report, and/or cases where number values are not available or are
zero. (4000 Character Limit)
Quarterly Emission Recall Progress Report (QR) Buttons
Add Quarterly Update - This button is used to for a Quarterly Update of multiple VERRs. Clicking this
button adds a new row at the end of the table to provide a Quarterly Update on another VERR. Delete
Last Quarterly Update - This button is used to delete the last Quarterly Update. Clicking this button
deletes the Quarterly Update Information in the last row of the table.
