Indicate how, by whom and for what purpose is this information used? The information will be used to determine customer satisfaction levels with current programs and explore ways to meet future needs of customers.

What consideration is given to use information technology to reduce burden? Consideration is given to collection techniques and the utilization of information technology with each individual survey to provide our customers additional options for submitting information electronically. In the past, surveys have been administered by email and on the web to minimize the burden to customers.
