COVER LETTER- ALREADY RECRUITED

Thank you for your interest in participating in the Salt Sources Study. You are scheduled for a study clinic visit on [INSERT DATE AND TIME IN BOLD TEXT]. The clinic is located in the Epidemiology Clinical Research Center at 1100 Washington Ave. South
Suite 201 Minneapolis, MN 55415. A map and directions to the building are attached to this letter. Free parking is available outside the building entrance (park in the area with visitor parking signs).


The clinic visit should take about 1 hour. The following activities will occur at this visit:

  • Review and sign consent form

  • Orientation to study activities

  • Complete survey that includes questions about salt use and demographic questions (e.g. age, sex, race, education level, etc.).

  • Height and weight measurement (in clothing with shoes, hat, and coat removed)

  • Schedule telephone interview and salt collection days

  • Schedule urine collection days, and days study provided salt will be used if you are participating in those study measures

  • Instructions and materials for study activities such as salt collection kits for the days samples of salt used in home cooking and at the table are to be collected.


I have enclosed a study fact sheet that provides an overview of the study. Please review this before your scheduled clinic visit.
