Dear John Smith:
Thank you so much for enrolling in the GuLF STUDY and sharing your story! Almost 33,000 people joined
the study, making it the largest oil spill health study ever conducted. Your ongoing participation will help
answer questions about any long-term effects of the spill.
The most important way you can stay involved is to complete a follow-up telephone interview. The
interview includes questions about your health and other experiences since we last spoke with you. This
is the main way we can learn about how oil spills affect health. It will take about 30 minutes to complete.
Participation is voluntary. The information you share is confidential and will be kept private. To show our
appreciation, we will enter you into a drawing for a $500 gift card after you complete the interview. If you
call our toll-free hotline and complete the interview within three weeks of receiving this letter, you will be
entered in a second drawing for an additional $500 gift card.
Keep sharing your story – do it for your health, your community, and your future. We will be calling you
in the next week or so to complete the interview, if you do not call us first. You can call our hotline toll-free

$500 Gift Card Drawing Details:
All participants who complete the follow-up telephone interview will be entered into a drawing for a $500.00 gift card.
Participants who complete the interview within three weeks of being mailed this letter will be entered into a second

drawing for a $500.00 gift card. Drawings will be held after every 500 participant completes the telephone interview.
One winner will be selected at each drawing. The odds of winning are 1 in 500 for all drawings. There is no cost
associated with entering the drawing or accepting the gift card. If you are selected as a winner, you will receive your
gift card in the mail.
