o Containing all 5 saliva samples in sealed plastic bag
 Large manila envelope labeled ‘Saliva – Day Two’
o Containing all 5 saliva samples in sealed plastic bag
 Saliva Collection Logs
Mail Instructions:
 Make sure envelope contains all the above contents
 Simply drop in any mailbox (Postage is already paid)
