Dear Ms. Dortch,
ABC Telco hereby accepts $7,750,000 of its eligible funding for Connect America Fund Phase I incremental support for 2012.1 To satisfy its deployment obligation in connection with this funding, ABC Telco will deploy broadband to 10,000 locations.
Please see the attached comma separated values file, which provides the following information about each location to which ABC Telco will deploy in satisfaction of this requirement: 2010 Census Block FIPS Code, NECA Company Code (OCN), Study Area Code, Wire Center CLLI Code, Latitude, and Longitude.

Pursuant to 47 C.F.R. § 54.312(b)(3), the undersigned certifies that (1) the locations that will be served in satisfaction of the deployment requirement associated with these funds, identified in the attached file, are shown as unserved by fixed broadband on the current version of the National Broadband Map (last confirmed on June 25, 2012); (2) that, to the best of the knowledge of ABC Telco, these locations are, in fact, unserved by fixed broadband; (3) that ABC Telco’s current capital improvement plan did not already include plans to complete broadband deployment within the next three years to the locations to be counted to satisfy the deployment requirement; and (4) that this incremental support will not be used to satisfy any merger commitment or similar regulatory obligation.
