This is a request for a nonmaterial/non-substantive change to OMB Control No. 0920-0199 Import Permit Applications, expiration January 31, 2017 used by the Centers for Disease Control and Prevention (CDC), Import Permit Program. Since the nonmaterial/non-substantive change only consists of changing the fax number, this nonmaterial/non-substantive change will not affect the burden in completing the form.
