Re: Nonmaterial/non-substantive Change to OMB 0920-0914

We are requesting a nonmaterial/non-substantive change to our OMB Approved Information Collection titled ‘Workplace Violence Prevention Programs in NJ Healthcare Facilities’ (OMB control number 0920-0914) with expiration date 2/29/2016. In order to re-initiate the recruitment process for hospital non-responders, we would like to request the follow changes to Appendix D (Letter of Introduction and Fact Sheet):

• Update the language in the letter of introduction to reflect the past recruitment activities and continued interest in scheduling an interview.

• Add the project coordinator’s name (Maryalice Nocera) and contact information to the letter of introduction.

• Update the language in the fact sheet to make the information clearer and add the project coordinator’s name (Maryalice Nocera) and contact information to the document.

The requested changes will not change the scope of the study and the burden hours will not change.
