Change Description

Reason for Change

Removed an extra box on A0050 response
field.

This was an error. The response choices are single digit,
so there should only be one box.

Change response 05 for item 1802 (Admitted
From) from “Short-stay acute hospital (IPPS)” to
“Short-stay acute hospital.”
Change response 07 for item A1802 (Admitted
From) from “Inpatient rehabilitation hospital or
unit (IRF)” to “Inpatient rehabilitation facility or
unit (IRF).”
For F3000 (Spiritual/Existential Concerns),
where “patient/caregiver” appears, change to
“patient and/or caregiver.”

Corrected the numbering in the “Skip to”
statement for F3000.
“The patient’s pain severity was” will now
precede “Type of standardized pain tool
used.” The numbering will be updated to
reflect the switch (J0900C and J0900D,
respectively), and a skip pattern will be
added if the patient’s pain severity is
“None.”
“Type of standardized pain screening tool
used” changed to “Type of standardized
pain tool used.”

Revised to make the responses more accurate.

This was an error that was raised in a public
comment. The skip pattern instructs abstracters to skip
to an item number that does not exist (I0100) rather
than I0010.
The measure steward felt that this would improve flow.
There were several public comments regarding the Pain
Screening item, although not directly aimed at this
ordering.
