Date when tagging takes place.
Number from tag attached to the hide.
Number from tag attached to the skull.
Village where hide and skull are tagged.
Usually left blank. This space is for an assigned number when a detailed necropsy is performed and
multiple samples are collected. A reference number here will allow us to relate samples to one animal.
Village otter was hunted from if different than the tagging village.
Estimated age class of sea otter: Adult – large size; Sub Adult – Intermediate size; Pup – small size,
young of year.
Sex of animal. Unknown when not absolutely sure.
How sex was determined.
Record biological specimens collected, preserved and presented to tagger.
How many otters were present at kill location and how many otters were harvested from that pod.
Record the date when the sea otter was killed; month, day, and year.
Identify specifically where this sea otter was killed. List distance from nearest island, river, bay, or
other geographic feature.
The GPS coordinates of kill location.
Enter any other information that may be important, like any unusual conditions or behavior of the otter.
The tagger signs here.
Print the name of the hunter.
The hunter or owner signs here.
After completing certificate, give pink copy to hunter, return white copy to our office,

NOTICE
Paperwork Reduction Act and Privacy Act
In accordance with the Paperwork Reduction Act of 1995 (44 U.S.C. 3501, et seq.) and the
Privacy Act of 1974 (5 U.S.C. 552a), please be advised that:
1. The gathering of information on marine mammals harvested by Alaska Natives is authorized
by :
(a) The Marine Mammal Protection Act of 1972 (16.S.C. 1379(i)); and
(b) Title 50, Part 18 of the Code of Federal Regulations.
2. Information requested on this form is required to obtain a benefit and comply with
statutory and regulatory requirements. This information is being collected to monitor the
subsistence harvest of sea otters and will be used to obtain essential biological data
necessary to manage the species. The collection of this information will also help control the
illegal take, trade, and transport of specified raw marine mammal parts. Response is not
required unless a currently valid Office of Management and Budget (OMB) control number
is displayed.
3. Routine use disclosures may be made:
(a) To the U.S. Dept. of Justice when related to litigation or anticipated litigation;
(b) Of information indicating a violation or potential violation of a statute, regulation,
rule, order or license to appropriate Federal, State, local or foreign agencies
responsible for investigation or prosecuting the violation or for enforcing or
implementing the statute, rule, regulation, order or license;
(c) From the record of an individual in response to an inquiry from a Congressional
office made at the request of that individual (42 FR 1903; April 11, 1977)
4. All names will be removed when we compile results, and only summary information will be
reported.
5. The relevant burden to report information required on this form and have the specified parts
marked or tagged is 15 minutes. Direct comments regarding the burden estimate or any
other aspect of the form to Service Information Clearance Officer, Fish and Wildlife Service,
Mail Stop 222, Arlington Square, U.S. Department of the Interior, 1849 C Street, NW,
Washington, D.C. 20240.

privileged and confidential business information to allow the Service to meet its responsibilities
