CONUS EDUCATION OPTIONS ASSESSMENT

ORAL CONSENT FORM

(For LEA superintendents)

RAND is a non-profit research organization based in Santa Monica, California. The Department of Defense has asked RAND to examine options for primary and secondary schooling of military dependents on each of the 15 continental United States (CONUS) installations where the Department of Defense currently operates Domestic Dependents Elementary and Secondary Schools (DDESS) or contracts with local educational agencies (LEAs) to operate schools on the installation. As part of this effort, we are interviewing key education stakeholders at each of these installations, including local LEA superintendents. In this interview we'll ask questions about your district and how you might approach the responsibility of educating military dependents currently served by DDESS schools.

Your participation in this discussion is entirely voluntary. You can choose not to do the interview or skip any points you would rather not discuss. But we hope you'll take part, because it is important to us to hear your insights. Your responses will help the DoD make decisions about how to provide schooling for military dependents including at this installation.

In our findings, we will not associate comments with specific individuals. We may paraphrase things you said, but we will not indicate that they came from you. We may associate comments with this installation.

If you would like more information about this study, you may contact any of the RAND project leaders: Charles Goldman, Rita Karam, or John Winkler at (310) 393-0411. For information on your rights as a human subject of research, please contact: Jim Tebow at (310) 393-0411 x7173 or [email protected]
